Check out this video of Austrian base jumper, Maria Steinmayr, narrowly escaping death after her parachute basically collapses mid-flight.

Steinmayr made the jump off the top of the Gran Bali Hotel in Spain and everything seemed fine as the chute opened. But then it collapsed, Steinmayr gets slammed into the building, it half opens and then shuts again.
